This Privacy Policy explains how Hydro Glo Health PLLC ("Hydro Glo Health," "we," "us," or "our") handles information collected through our website, hydroglohealth.com.

Who we are

Hydro Glo Health PLLC is a nurse-led practice in Charlotte, North Carolina. Our contact details are at the end of this policy.

What this policy covers

This policy covers our website only: what information it collects, how we use it, and who we share it with.

Patient health information is handled under our separate HIPAA Notice of Privacy Practices. You can request a copy using the contact details at the end of this policy.

Information you give us

Eligibility quiz. Our medical weight-loss page has a 60-second eligibility quiz. It asks for:

• Health screening answers: your goal, whether you are 18 years of age or older, your BMI range, whether you can come in, certain health conditions (pregnancy or trying to become pregnant, breastfeeding, a personal or family history of medullary thyroid cancer or MEN2, a history of pancreatitis), and any prior use of compounded semaglutide or tirzepatide.
• Contact details: your name, email address, and phone number.
• Your consent to be contacted, through a checkbox that reads: "I agree to be contacted by Hydro Glo Health about my consultation by call, text, or email. Message and data rates may apply. Consent is not a condition of care."

Each quiz submission also includes:

• the consent wording you agreed to;
• whether your answers passed the screening;
• the address of the page you submitted from, with any referral details removed; and
• the time of submission.

When you submit the quiz, your submission is sent:

• to us by email, through an email delivery service (Resend); and
• to our LeadConnector (HighLevel) account.

Our website does not keep a database copy of quiz submissions.

Booking. Our booking buttons and scheduling links open booking pages run by third-party services, described below. Information you enter when booking is used to schedule and coordinate your consultation.

Calls and texts. When you call or text us, we receive the information you choose to share.

Payments. Our website takes no payments and has no payment form.

Information we collect automatically

• Hosting. Our website runs on Cloudflare (Cloudflare Pages). Like any web host, Cloudflare processes request details, such as your IP address and browser details, to deliver and protect the site.
• Abuse protection. To slow repeated submissions, the eligibility quiz keeps a temporary count of submissions from each IP address. Once a count reaches 8 within an hour, the quiz may refuse more. We drop each count after one hour.
• No cookies and no analytics. This site sets no cookies and runs no analytics. It uses your browser's session storage only to hold an active referral code (see Referral links below).

Referral links

Some links to our website carry a referral code. When you open one, our website stores the code and the time it was captured in your browser's session storage. Session storage is cleared when you close the tab.

While the code is active, our website:

• shows a note that a friend shared the link;
• adds the code to our links to the LeadConnector booking pages and to our practice scheduler at my.hydroglohealth.com; and
• marks any quiz submission as joining with a friend, and includes the code and the time it was captured.

From the link itself, our website stores and sends only the referral code. It does not store or send a friend's name or phone number.

How we use information

We use information collected through our website to:

• review your quiz answers;
• contact you about your consultation by call, text, or email;
• schedule and coordinate your consultation and care; and
• protect the website against spam and abuse.

How we share information

We share information collected through our website only as this policy describes:

• with the email delivery service and the LeadConnector account that receive quiz submissions; and
• with Cloudflare, which hosts and protects our website.

We may also disclose information when required by law. We do not sell personal information collected through this website.

Third-party services

Our website links to services run by other companies:

• LeadConnector booking pages, opened by our "Book a Consultation" buttons.
• Our practice scheduler at my.hydroglohealth.com, opened by our weight-loss scheduling links.
• Our profiles on Facebook, Instagram, and LinkedIn.
• Google Maps, for directions.

These are links, not content embedded in our website. Each service has its own terms and privacy policy, which apply when you use it. This policy does not cover those services.

Our website also links to other websites, including those of partner businesses. They are run by others, under their own terms and privacy policies. We are not responsible for their content or practices.

Text messages

If you provide a mobile number, you agree to receive appointment-related text messages. Consent is not a condition of care.

Reply STOP to opt out, HELP for help. Message and data rates may apply. Mobile numbers are not shared for marketing.

Security

• Our website is served only over HTTPS, an encrypted connection.
• The eligibility quiz accepts submissions only from our own pages and limits the size of each field.
• Quiz submissions are sent to our email delivery service and to our LeadConnector account over encrypted (HTTPS) connections.

No method of sending or storing data over the internet is completely secure.

How long we keep information

• Quiz submissions are held in our email and in our LeadConnector account, not in a website database.
• We keep quiz submissions as long as we need them to respond to you and coordinate your care, or as the law requires.
• Referral details in session storage are cleared when you close the tab.
• The count used to limit repeated submissions is dropped after one hour.

Your choices

• To ask about or update information you gave us through our website, contact us by phone, text, or mail.
• To stop text messages, reply STOP at any time.
• The eligibility quiz is optional. You can call or text us instead.

Children

Our website is for adults 18 and over. We do not knowingly collect information from anyone under 18.

Changes to this policy

We may update this policy from time to time. We post any update on this page with a new effective date.
